We are seeking a suitably qualified Registered Nurse or Clinical Nurse Consultant to become a part of the Western Health Bacchus Marsh Community Palliative Care Team. This position requires a Registered Nurse with recent (within the last 12 months) or current experience in Specialist Palliative Care who has excellent assessment and problem-solving skills for the care of consumers with life limiting conditions in their homes. The role requires an experienced nurse who can prioritise patient care, escalate patient care and be flexible to meet the demands of a changing day to day business.
The successful applicants will be required to have well developed clinical and communication skills. This position is offered on a fixed term, part-time basis (0.4 FTE) until 12 February 2027, with set days weekly.

We are seeking someone with:
- The ability to provide evidence-based patient centered care
- The ability to prioritise and escalate patient care
- The ability to work collaboratively within the nursing team
- Recent (within the last 12 months) or current experience in Specialist Palliative Care
- Well-developed clinical and communication skills
- Excellent assessment and problem-solving skills
