Seniors Month Webinar Series 2025
Over the month of October, Palliative Care Victoria will be delivering a four part webinar series to celebrate Seniors Month.
Following the success of last years series, this years topics will explore how older Victorians can access and navigate palliative care alongside aged care services. In each session we will feature presentations from industry experts who have generously volunteered to share their insights in plain, easy to understand language. Four webinars will be held throughout the month of October on Friday afternoons from 3pm to 4pm, with a different topic each week.

Session One
October 3: Accessing Palliative Care in Aged Care
In this session, we will look at the different pathways an older person may take when accessing palliative care support. The key topics covered will include:
- Transitioning from hospital to an aged care service for palliative care
- The referral process to community palliative care services
- How aged care services collaborate with other health professionals to deliver care
Session Two
October 10: Palliative Care in the new Aged Care Act
This session will unpack what the new changes to palliative care are under the new Aged Care Act. In this discussion we will look at what these changes mean people receiving care under a homecare package as well as within an aged care home and highlight several key resources available to support aged care services and the community.
Session Three
October 17: Advance Care Planning in Aged Care
Advance Care Plans and Directives have become a hot topic in recent years, particularly for people entering aged care. In this session we will be joined by the experts at Advance Care Planning Australia to talk to:
- What information is included in Advance Care Plan
- Why they are so important to aged care services
- How are they recorded and where people can get help completing one

Session Four
October 24: Palliative Care and Dementia
Dementia is now the leading cause of death in Australia. Building an understanding of the disease and preparing early for its progression can significantly improve the quality of life a person experiences in their final days.
The key topics we will cover in this session include:
- How dementia can progress and care needs change
- Communication tips when supporting a person living with dementia
- How to prepare and plan ahead for palliative care
- What support is available for families and carers
