About Cabrini's Palliative and Supportive Care

Cabrini’s palliative and supportive care service provides compassionate and expert care to patients living with life-limiting illnesses, receiving end-of-life care or requiring support with pain and symptom management during or after treatment. The purpose-built palliative and supportive care inpatient unit at Prahran houses 22 single rooms, and is supported by a team of skilled doctors, nurses, pastoral and social workers, chaplains, counsellors, dietitians, pharmacists and physiotherapists.

About the role

A wonderful opportunity has arisen for a highly motivated and enthusiastic nurse to join a dedicated team of multidisciplinary palliative care experts. If you are a dynamic and suitably qualified Registered Nurse that has proven leadership skills - this is your opportunity to join our team! This position will suit an registered nurse who has a passion for providing evidenced-based palliative care and has the ability to lead a team and coach staff to deliver an exceptional patient and family experience.

The successful candidate will be a team player, use a strength-based approach to leadership and work closely with other Afterhours Coordinators.

The role reports to and is supported by the Nurse Manager Cabrini Prahran Inpatient Unit.

Key responsibilities will include:

  • Coordination of the shift and planning for the forthcoming shift and patient activity
  • Provides clinical expertise for the clinical team after hours and on weekends
  • Communicating and escalating issues to appropriate senior staff as per policies and procedures and ensuring follow-up
  • Providing support and coaching staff when required to assist in problem solving
  • Liaising with medical staff and other visiting health professionals to facilitate an integrated model of care
  • Triage community calls documenting clinical information and outcome
  • Liaise with the Homecare team regarding out of hours visits and admissions

Selection Criteria

About you

Along with your passion for palliative care nursing, you will have the following:

  • Relevant nursing qualifications and current registration with AHPRA as a Registered Nurse
  • Post graduate qualifications and/or previous experience in Palliative Care or relevant stream
  • Proven ability to lead and motivate a team
  • An interest/experience in driving quality improvement activities and ensuring outcomes are delivered
  • Demonstrated ability to perform effectively under pressure and prioritise tasks
  • Excellent customer service with attention to detail
  • Excellent computer skills with proficiency in Microsoft Word, Excel and Outlook
