Summary
The Monash Health Supportive and Palliative Care Service provides a bi-disciplinary Palliative Care Consultancy Service and works in close partnership with McCulloch House, a Level 3, 16 bed Inpatient Palliative Care Unit and 5 inpatient beds within a general medicine ward at Casey Hospital. We are a supportive and well-established team within a large tertiary metropolitan health service seeing on average 2800 patients per year. In addition, this service contributes to teaching, education and research efforts within Monash Health, and in collaboration with Monash University.
We are seeking an accomplished and motivated nurse to join our RAPID service(Rapid Palliative Care InReach Division) as part of the Palliative Care Consultancy Service at Monash Health.
The role is 0.7 EFT, 12 months fixed term, Monday - Friday 0800 - 1630. There is some flexibility in working days.
This position will extend the current model of care, designed to complement the existing inpatient bed-based services and consultation activity, including innovation and integration of home-based care. This model seeks to improve continuity of care between inpatient and community palliative care to address any unstable clinical issues, via a rapid response approach.
Selection Criteria
- Current AHPRA registration Post graduate qualifications in Palliative Care speciality
- Preferable Community Palliative care experience
- A proven commitment to the principles of palliative care and evidence based practice
- High level of computer literacy
- Current driver’s licence and own vehicle with ability to work across multiple Monash Health sites as required
- Applicants new to Monash Health must provide evidence of immunisation before they can receive an offer of employment and will be required to enter into an Employment Agreement prior to commencement
