Summary
We are excited to offer this Social Worker opportunity to join our Specialist Community Palliative Care team.
About the Team
The BCH Palliative Care service comprises an interdisciplinary team of a Nurse Practitioner, Specialist Community Palliative Care Nurses and a Social Worker. We work in conjunction with BCH Community Nurses and Occupational Therapists with access to Specialist Medical support.
About the role
The Palliative Care Social Worker will work in partnership with the Palliative Care team and our clients, to achieve positive outcomes that optimise health and wellbeing in accordance with their needs, goals and preferences. In doing so, this will help our clients to live safely and independently at home. The role of Social Worker includes oversight of the provision of evidence based and goal orientated services for BCH Palliative Care clients.

Selection Criteria
We would like to hear from you, if you ...
- Hold tertiary qualifications in Social Work, Psychology or Counselling.
- Have current registration with the relevant Professional Association.
- Have an excellent understanding of consumer directed care within a palliative care framework.
- Have a demonstrated ability to deliver person centered care with issues associated with dying, death and bereavement.
- Are competent in the use of IT systems, including MS Office and Client Management Systems.
