Summary
Palliative Care South East (PCSE) provides palliative care in the south east metropolitan region of Melbourne. We support people with a life-limiting illness to live well and, in doing so, provide support within the community to improve the quality of life of individuals, carers and families. Palliative Care South East provides a 24 hour service 7 days a week within the local government areas of Cardinia, Casey, Greater Dandenong and part of Kingston.
About the Role
The Aged Care Nurse Consultant (ACNC) role is focused on the provision of specialist palliative care either directly or through support and guidance to staff employed in various residential aged care facilities located within the PCSE catchment. The ACNC will also assist Residential Aged Care Facility’s (RACF) to develop resources and processes that will better support the implementation of a palliative approach in line with contemporary evidence-based practice guidelines for a palliative approach in RACFs.
Some of your responsibilities will include:
- Provide clinical review of clients within residential aged care facilities who have complex palliative care needs referred to PCSE services
- Maintain effective communication with residential aged care facilities involved in providing care to palliative care clients
- Provide current information to clients’ families and carers and ensure access to appropriate services at initial assessment and as subsequently identified or upon request
- Develop and maintain a collaborative approach with local GPs who provide primary medical support for palliative clients in residential aged care facilities
- Assist with the introduction / implementation of Advanced Care Planning practices policies and processes
- Provide education opportunities aimed at capacity building for residential aged care facility staff in provision of palliative care/palliative approach to all residents

Selection Criteria
- Registered Nurse (Division 1)
- Experience in an aged care service
- Demonstrated palliative care experience
- A postgraduate qualification in Palliative care, Aged Care or Gerontology is highly desirable
- Current registration with Australian Health Practitioner Regulation Agency (AHPRA)
- Excellent communication and interpersonal skills both written and verbal
