About The Role:
Are you a skilled Registered Nurse with experience in palliative care? Do you enjoy networking and developing relationships with other services, carers and people with chronic life-limiting illness? Be part of a community owned, purpose driven service set in a picturesque rural South West Victoria location. Key requirements include:
- Capactiy to work independantly as well as part of a team
- Excellent oral and written communication skills
- Promote and enable relationships with the wider community to inform and provide an understanding of our service
- Build relationships with other health service providers, GP clinics, Allied Health Professionals and Case Managers, along with potential service users and carers
- Further develop our reputation for excellence in respite and end-of-life care
- Co-ordinate and facilitate illness specific support groups
- Plan, implement and evaluate community events such as open days
