Summary

Peninsula Home Hospice is a not for profit community palliative care service providing support to clients in their homes. Our catchment area is the Mornington Peninsula, Frankston and part of the City of Kingston.

We are seeking a compassionate Client Resource Advocate with excellent communication skills and a dedication to making a difference for clients and families quality of life. Whilst working as a member of an interdisciplinary team, you will support PHH clients to have equitable access to information and resources. You will maintain effective relationships with external service providers in collaboration with the clinical team. The position requires established work practices that help to maintain systems that support needs based resource allocation whilst meeting funding agreement requirements.

Selection Criteria

What you'll bring

• Tertiary qualifications in Social Work, Welfare or Community Development are mandatory requirements for this role. 
• Ability to work within an interdisciplinary team 
• Eligibility for membership or registration to relevant professional association 
• Extensive experience and high skill level for client advocacy 
• Demonstrated ability to work with multiple service providers
