Summary

This is a 12-month fixed-term contract (maternity leave cover), requiring a commitment of 0.9 FTE, starting in March 2024.

In this role you will work closely with the multidisciplinary team to provide comprehensive support to individuals and families. Your role involves taking a holistic approach to client and family work, with expertise in the psychological, social, and emotional dimensions of end-of-life.

Your responsibilities include conducting episodic casework based on internal referrals, leveraging your expertise to assess social determinants of health, and delivering psychosocial interventions. You'll develop tailored care plans to enhance the quality of life for clients and families, assisting them in navigating healthcare systems and accessing necessary resources.

Selection Criteria

To be successful, you will have:

  • Bachelor of Social Work Must be eligible for membership with the Australian Association of Social Workers (AASW) 
  • A postgraduate qualification in Palliative Care is desirable 
  • Demonstrated knowledge of the principles and practice of social work within palliative care, such as grief and bereavement support, as well as complex medico-ethical-legal issues 
  • High level ability to communicate verbally and in writing in an articulate and concise manner 
  • Demonstrated ability to network and build relationships with key external service providers involved in social support services 
  • Ability to lead groups and facilitate meetings with families and other agencies 
  • Demonstrated ability to provide care in partnership with key external service providers 
  • Commitment to the preservation and empowerment of disadvantaged and marginalised 
individuals and families, including those from Culturally and Linguistically Diverse communities 
  • Commitment to social justice principles, and specifically to the mission and aims of PCSE 
  • Computer literacy – Microsoft Office suite and clinical electronic databases
