Peninsula Home Hospice (PHH) is seeking an experienced Nurse Practitioner (Palliative Care) to join our specialist community palliative care service, supporting people living with life-limiting illness across the Mornington Peninsula and surrounding areas to live well and die well. For over 40 years, PHH has provided compassionate, person-centred care in the place people call home, supporting dignity, comfort and quality of life for clients and families.
This is a permanent, part time (0.8) role based from our beautiful offices in Mornington. This senior clinical role will involve providing advanced assessment, diagnosis and management of complex palliative care needs across community, telehealth and residential aged care settings. You will work autonomously within your endorsed scope of practice while also providing clinical leadership, mentoring, education and contribution to quality improvement and service development. A key focus of the role is supporting clients and families through advance care planning, symptom management and end-of-life care.
Selection Criteria
You are an experienced palliative care clinician with strong advanced clinical judgement, excellent communication skills, and a compassionate, person-centred approach. You thrive in a collaborative environment and are confident working within a multidisciplinary team.
Key Requirements:
- AHPRA registration as a Nurse Practitioner with NMBA endorsement
- Master’s degree and postgraduate qualification in palliative care
- Extensive experience in palliative care and complex symptom management
- Experience in community or home-based care
- Strong skills in advance care planning and end-of-life discussions
- Clinical leadership and mentoring capability
