We are looking for an EEN or RN to fill our Clinical Care Coordinator role, based in our office at Camberwell Junction, close to all public transport. Call on Clare works in partnership with our clients, medical professionals, and other allied health services to coordinate care based on respect, preserving dignity and choice, and assisting clients to maintain quality of life and independence in the community.

We are looking for a person who has experience or is currently working in a community setting dealing with both low and complex care needs and has an understanding of the requirements for clinical assessments in the community. This role is office-based and you shall be providing support for our nurses and carers in the field, our clients, and our medical teams.
The Role will Involve:
- Providing support for our nurses and carers in the field
- Consult with clients and their families to assess care and support needs
- Prepare service plans and work with the scheduling team to allocate appropriate care workers for clients to provide continuity of care
- Conduct assessment and reassessment of Care Plans for clients
- Regularly review client records to ensure all care is being provided as per care plan
- Responsibility for a caseload of clients; Care planning, responding to client and staff feedback, liaising with relevant services and individuals
- Ensuring that clients’ rights of dignity, respect and religious/cultural beliefs are upheld in accordance with relevant legislation and standards
- Liaising with external organisations who are utilising Call on Clare’s brokerage services, e.g. Eastern Palliative Care, Cabrini Hospital
- Following up client enquiries to achieve the highest possible enquiry conversion rate
- Conceiving and implementing processes and initiatives designed to enhance the client engagement process
- Respond to any concerns regarding safety, quality and risk including near misses and injuries
- Assist with the training, coaching & support of nurses and support workers

Selection Criteria
- AHPRA Registration
- Minimum of 3 years experience in a community setting
- Experience in providing palliative care or community nursing is highly regarded
- Experience in a Clinical setting or Clinical Care Coordinator role is highly regarded
- Exceptional customer service and clinical care
- Excellent communication skills with a compassionate and empathetic approach to your work
- Excellent communication skills
- Good computer skills
- Outstanding organisational, problem-solving and multitasking skills
- Demonstrated ability and flexibility to assess and respond to changing priorities in a fast-paced environment
