We are seeking a compassionate and skilled Social Worker (Grade 2 or Grade 3) to join our Community Palliative Care team. This role provides psychosocial assessment, counselling, advocacy, and care coordination for individuals, families, and carers affected by life-limiting illness and bereavement. Working as part of a multidisciplinary team, you will support quality of life, dignity, and informed decision-making across the palliative care continuum.

Selection Criteria

You would require the following to be eligible:

  • Recognized bachelor’s or master's degree in social work and eligible for registration as a Social Worker with The Australian Association of Social Work (AASW). 
  • Minimum of five years’ experience as a Social Worker, with at least three (3) years’ experience in palliative care or related fields including: oncology, chronic conditions, mental health, or aged care. 
  • Ability to lead advanced psychosocial assessment and intervention across the palliative care continuum, managing highly complex, high‑risk presentations and providing expert consultation to the multidisciplinary team 
  • Ability to provided senior-level coordination and advocacy to address barriers to care, supports vulnerable populations, and leads crisis and risk management. 
  • Ability to provide clinical supervision, specialist bereavement support, and advanced education for patients, families, and staff. 
  • Contributes to culturally safe practice, service development, governance activities, and the training of students and early‑career practitioners
